Short stem hip replacements are salvage for failed hip resurfacing

Metal on metal hip replacements and hip resurfacings have come under a lot of flak in the last few years. Many patients were taken in by the marketing hype and underwent these procedures. Now these implants have been failing at increased rates. These patients face the prospect of a revision hip replacement. Short stem hip replacements or neck serving hip replacements are an option in these young patients. They preserve valuable bone. Early diagnosis with serum chromium levels, MRI scans can detect raised metal levels and loosening. Early conversion to short stem hip replacements can avoid further complications including that of the dreaded 'C' word. Harder and safer bearings like ceramic, polyethylene can be used instead of metal on metal.

AVN treatment in India- Stem cells, Mini hip replacement

AVN or avascular necrosis of hips is a common condition in India. The Times of India recently reported that there are 20 cases in 100000 of population. This is a sizeable number. It is a condition where the blood supply to the femoral head (uppermost part of thigh bone)gets shut off and causes the bone to collapse slowly. As the bone dies, the patient experiences excruciating pain and limited or no movement. Late cases can lead to arthritis of the hips.Stem cell treatment in early cases is a latest advance and has been proposed to prevent the disease from advancing further. If it does so and causes bone collapse,traditionally a total hip replacement has been recommended. We advocate a more conservative approach with a short stem hip replacement. Custom made blocks enhance outcome of knee replacement in Chennai

Patients seeking a knee replacement at our facility can now avail of a latest technology. We are offering patient specific instrumentation (custom made blocks). Revision hip replacement relieves Ugandan midwife's agony

A revision hip replacement was performed for a retired Ugandan midwife. A cemented hip replacement done in 2004 had loosened. She was unable to give up her crutches since the operation. She had severe bone loss in the femur and moderate bone loss in the acetabulum. Dr.A.K.Venkatachalam performed an uncemented revision hip replacement. He used the Wagner stem and Trilogy cup from Zimmer. Allograft bone was used to reconstitute acetabular and femoral bone loss.
The same patient had undergone a primary knee replacement last week.
Thousands of patients from Afrcian countries travel to India for joint replacements. Dr. Venkatachalam is a joint replacement orthopedic surgeon.

Ceramic hips are boon to young patients

Ceramic hips were introduced by Dr.A.K.Venkatachalam of Madras Joint Replacement center in December. This material has the capacity to last longer than metal bearings. The first patient to receive a ceramic hip replacement was a 25 year old male from Trichy called Nizam. He was suffering from a condition of his hip called 'Protrusio acetabuli'.

Avacular necrosis, ankylosing spondylitis, protrusio acetabuli, dysplasia, Perthes disease, septic arthritis are some of the conditions compelling young patients to go in for hip replacement in India. Since all these patients are young we need hard bearings like Ceramic or metal. Metal resurfacings which gained popularity in the last decade fell in popularity over the second half of 2010 due to adverse effects. During the last decade we saw the rise and fall of metal bearings in Hip resurfacings and metal on metal hip replacements. A medical device company Depuy was forced to recall its Hip resurfacing and replacement products ASR from the market. In India too, many young patients were falsely lured into this procedure by hyped up advertisement.
Over a short period of time many such patients developed allergic reactions, metal induced wear and loosening. They had to undergo a painful second operation.
The next decade of the 21st century is going to see ceramics play a big role in joint replacements particularly of the hip.

Ceramics have been in use in ship propellers, air craft for many years. Now a new class of ceramics called Delta ceramic has been introduced in artificial joints. These are more stronger and less brittle.
They can survive for very long periods in the body,
says Dr.Venkatachalam. Incidentally Dr. Venkatachalam was the first surgeon to launch Ceramic coated knees or Oxinium knees in South India two and half years back. The first patient was a 34 year old Dutch patient with end stage knee arthritis. Since then many young patients have undergone Oxinium knee replacement by this surgeon.
This Ceramic hip replacement is pure ceramic unlike the knee which is Ceramic coated Oxinium.
